Students who wish to participate on a sports team MUST have had a physical exam within the last 12 months.  Students will NOT be permitted to participate until our school doctor has performed a physical exam or reviewed a copy of a physical from home.

Also, an updated health history is required to be eligible (to be completed by parents-NO MD signature required).

Any students who require medication scheduled or as needed (such as inhalers) MUST have updated supply of med AND MD Order sheets completed by physicians from home

ALL Forms need to be completed and returned in a timely manner.

The school physician will be scheduled upon receipt of forms.
